Cumberland County Program Targets Chronic Medical Issues

Cumberland County Health Department is providing a Chronic Disease Self-Management Program from Stanford University called “Take Control of Your Health.”

The Chronic Disease Self-Management Program is recognized worldwide as a cost-effective evidence-based health promotion program for adults with chronic conditions and the caregivers of individuals living with a chronic condition.

Cumberland County Program Targets Chronic Medical Issues“Take Control of Your Health” provides workshops for a small group of adults of all ages that meet for 2 1/2 hours, once a week for six weeks. The 6-week workshop allows the group of participants to help one another with solving problems that affect their health condition.

Trained certified Chronic Disease Self-Management leaders will help participants manage chronic health conditions such as diabetes, high blood pressure and arthritis during each workshop. The interactive workshops will help people deal with issues they experience with their chronic health condition such as fatigue, pain, breathing issues, sleeping problems, loss of energy and depression.

Workshop schedule: Tuesday, September 20 to Tuesday, October 25, 1:00 p.m. to 3:30 p.m. at the Cumberland County Health Department, 309 Buck Street, Millville.

Each participant will learn healthier ways to live while maintaining an active and fulfilling life. They will also gain confidence and motivation in managing their health by:

  • Communicating more effectively with their doctor
  • Reducing frustration
  • Fighting fatigue
  • Making tasks easier
  • Getting more out of life
